The Basti therapy alternates between Matra Basti (oil enema) and Niruha Basti (decoction enema). Could the authors clarify the rationale behind this sequence? Was it based on prior clinical studies, or is there an Ayurvedic textual reference supporting this approach?

The patient had previously used NSAIDs before switching to Ayurveda. Could the authors discuss whether a placebo effect might have contributed to symptom improvement? Would a comparative study (Ayurveda vs. NSAIDs or physiotherapy) provide stronger validation of the treatment efficacy?
